Learning objectives: Understanding normal breast development enables radiologists to recognize pathologic conditions that require further work-up or pathological confirmation. This educational poster examines physiologic imaging changes of the paediatric breast and seeks to raise awareness for imaging features that require further investigation. The commonest benign and malignant paediatric breast lesions are reviewed.
Background: Palpable breast lumps in the paediatric population are often concerning for patients and family members, even though most findings are benign. Ultrasound is the imaging modality of choice due to its high sensitivity, ease of access and role in interventional procedures, while lacking the detrimental effects of ionizing radiation. Breast biopsies should be reserved to the most suspicious lesions, as the developing breast is highly vulnerable to iatrogenic injury and permanent aesthetic changes may result.
Findings: Breast pathology in children ranges from congenital anomalies to developmental and acquired conditions but, in most cases, the abnormalities found are due to variations in normal breast development. Inflammatory and tumoral conditions do occur, but malignancy is quite uncommon. The imaging features of physiologic breast development will be reviewed, taking into account normal and abnormal findings on different age groups. Additionally, the commonest pathologies will be reviewed, such as gynecomastia, mastitis and fibroadenomas, among others.
Conclusion: Imaging plays a pivotal role in the accurate diagnosis and management of paediatric breast conditions and radiologists should be aware of the spectrum of normal findings to be able to correctly identify the commonest changes and determine the need for further investigation.
